Any and All Services will NOT be rendered until all necessary documentation is signed, dated, and returned.

The documentation for Physical Therapy services (In-Person and Telehealth) will be provided immediately after signing up through the patient portal once the service is paid for and booked. Failure to provide signed documentation will result in the paid-for service being withheld until all documentation is received, without possibility of refund. Documentation for Remote Training services will be sent to the provided email address once the service is paid for. Programs will not be delivered on the TrainHeroic application until the required documentation is returned. The links provided below are for the convenience of Site users to review and/or fill out prior to purchasing any services.
